Our registered manager will visit you at home or liaise with the multidisciplinary team caring for you at hospital in order to assess your needs and plan continuity of care at home.
A3 care will welcome referrals from GPs, Private Clients, Primary Care Trust (PCT), NHS, private hospitals, Social Services within Local Authorities, and the Local Authority.
If you are already receiving care services from another care provider and would like A3 Care to provide your care, you have the right to change. With your permission we will work with your existing provider to ensure that your care package transfers to us smoothly.
